Outpatient rehabilitation services are available at Baldwyn Wellness Center from 7:30 a.m. to 5 p.m. Monday-Friday. The facility is equipped to treat a wide variety of diagnoses from orthopedic injuries to complicated medical and neurological disorders. The staff has more than 35 combined years of experience and extensive knowledge in physical, occupational and speech/language rehabilitation techniques. Following discharge from rehabilitation services at the state-of-the-art fitness center, individuals often choose to continue their efforts by joining Baldwyn Wellness Center with a membership discount.

About Your Treatment

Upon referral from a physician, a speech, occupational, or physical therapy assessment is performed and a rehabilitation plan of care is established. Areas of expertise include treatment for:

  • Acute or Chronic Pain
  • Balance Disturbances
  • Brain Injury
  • Difficulty with Fine Motor Skills
  • Dysfunction
  • Gait Disturbances
  • Incoordination
  • Orthopedic Conditions
  • Other Neurological Conditions
  • Speech and Swallowing
  • Spinal Cord Injury
  • Sports Injuries
  • Stroke
  • Weakness

Services include ergonomic assessments, work conditioning programs, low vision treatment, diabetic foot care and other wound care. Treatment duration depends on the severity of the injury/dysfunction and the patient’s level of progress. We work closely with the patient and physician to provide the highest level of care available to assist with returning the patient to his or her prior level of function with as little downtime as possible.

Wound Care

Baldwyn Outpatient Rehabilitation is home to a Certified Wound Care Specialist and therapists who are skilled in performing debridement and wound care for:

  • Burns
  • Diabetic Wounds
  • Vascular Wounds
  • Trauma Wounds

Baldwyn Outpatient Rehabilitation offers whirlpool treatment, as well as pulsed lavage treatment and diabetic foot care.

Low Vision Rehabilitation Services

Services are provided by an occupational therapist with a graduate certificate in low vision rehabilitation for individuals with glaucoma, macular degeneration, cataracts, diabetic retinopathy, stroke and traumatic brain injury.

The therapist begins by evaluating the patient’s ability to perform daily tasks and the areas that have been affected by vision impairment. Training may be provided in either the patient’s home (if within a reasonable distance), at Baldwyn Outpatient Rehabilitation, or in some cases both.

Services may include training with optical and non-optical devices, paying bills and managing finances, managing medications, preparing meals safely, teaching use of remaining vision and teaching non-visual methods. Medicare and most insurance will cover low vision services when referred by a physician and provided by an occupational therapist.

A signed physician’s prescription dated within the last calendar year is required, along with a qualifying diagnosis necessitating the need for skilled therapy services.


Baldwyn Outpatient Rehabilitation accepts Medicare, Medicaid, private insurance and Workers’ Compensation as payment for services rendered. Pre-certification will be required for the majority of patients and will be performed by the NMMC Admissions Department.

Things You Should Know

Please bring your prescription/orders for therapy from your physician on your first visit, along with any other necessary paperwork, insurance cards/information, and a list of your medications. Appropriate footwear is recommended.

On your first visit to outpatient rehabilitation, plan to arrive 15 minutes early to allow for paperwork before your evaluation begins. If you will be late or cannot come for your appointment, please call so that we may make arrangements to reschedule. If we cannot make convenient arrangements, your therapy will continue at the next scheduled time.

If you miss three appointments in a row without notifying the department, you may be subject to discharge. You will be notified of any change made regarding your therapy. If discharged, you will need updated orders from your physician to resume therapy. If you are unable to complete or choose to discontinue your rehabilitation program, please contact the Rehabilitation Services Department.
