Dr. Erik Dukes photo

Erik Dukes, M.D.: Received a bachelor's degree from William Carey College in Hattiesburg. He completed his medical training at Louisiana State University School of Medicine and his family medicine residency at Floyd Medical Center in Rome, Georgia. He completed a sports medicine fellowship at the Center for Sports Medicine at Bayfront Medical Center in St. Petersburg, Fla. Dr. Dukes is a Diplomate of the American Board of Family Practice. In 2010, he received three-year recognition from the National Committee for Quality Assurance (NCQA)/American Diabetes Association (ADA) Diabetes Physician Recognition Program.

Sean Reynolds, M.D.: Received his bachelor's degree from Georgia Southern University in Statesboro, Ga., and completed his medical training at Mercer University School of Medicine in Macon, Ga. He completed his family medicine residency at Floyd Medical Center in Rome, Ga. Dr. Reynolds is a member of the American Academy of Family Physicians. In 2010, he received three-year recognition from the National Committee for Quality Assurance (NCQA)/American Diabetes Association (ADA) Diabetes Physician Recognition Program.

Jenna Dixon, PA-C: Received her bachelor's degree from the University of Southern Mississippi in Hattiesburg. She received a master's degree in medical science from Lincoln Memorial University-DeBusk College of Osteopathic Medicine in Harrogate, Tenn. She is a licensed physician assistant.

Abigail Moss, CFNP: Received her associate's degree in nursing from Northeast Mississippi Community College and her master's degree in nursing from Vanderbilt University. She is a certified DOT Medical Examiner and is listed on the National DOT Registry.

Our clinic is open Monday through Friday from 7:30 a.m. to 6:30 p.m.


Appointments may be made by calling (662) 728-2071 during office hours. Appointments may be requested with the health care provider of your choice. Every effort will be made to provide the earliest possible appointment for the convenience of the patient. Due to the unscheduled nature of emergencies, occasional delays do occur. We hope you will understand these delays are unavoidable.

If you are unable to keep your appointment, please cancel as far in advance as possible. Another patient who can be booked into the open time will be grateful for your thoughtfulness.

Patient Portal Offers Secure Online Services to Your Health Information

We now have the ability to exchange secure online correspondence with our patients and keep your information up-to-date. If you have Internet access and sign up for this service, you will have the ability to do the following at your convenience:

· View your patient chart summary

· Request an appointment

· Request a medication refill

· View test results

· Change personal information

· Access forms

To access the service, you will register using a PIN (Personal Identification Number) that will be provided by our clinic staff to you in a personalized letter after a visit to our clinic. The PIN will expire in 30 days. If your PIN has expired before you access your account, please visit us in person to obtain a new PIN. To ensure patient confidentiality, we do not provide this information over the phone.
