Medical Staff

Erik Dukes, M.D.: Received a bachelor's degree from William Carey College in Hattiesburg. He completed his medical training at Louisiana State University School of Medicine and his family medicine residency at Floyd Medical Center in Rome, Georgia. He completed a sports medicine fellowship at the Center for Sports Medicine at Bayfront Medical Center in St. Petersburg, Fla. Dr. Dukes is a Diplomate of the American Board of Family Practice. In 2010, he received three-year recognition from the National Committee for Quality Assurance (NCQA)/American Diabetes Association (ADA) Diabetes Physician Recognition Program.

Sean Reynolds, M.D.: received his bachelor's degree from Georgia Southern University in Statesboro, Ga., and completed his medical training at Mercer University School of Medicine in Macon, Ga. He completed his family medicine residency at Floyd Medical Center in Rome, Ga. Dr. Reynolds is a member of the American Academy of Family Physicians. In 2010, he received three-year recognition from the National Committee for Quality Assurance (NCQA)/American Diabetes Association (ADA) Diabetes Physician Recognition Program.

Amber Janzen, CFNP: completed her nurse practitioner training at Mississippi University for Women in Columbus. Janzen completed undergraduate degrees at Northeast Mississippi Community College in Booneville and the University of North Alabama in Florence. Janzen is a member of the Mississippi Nurses Association, American Academy of Nurse Practitioners, and Sigma Theta Tau International Nursing Honor Society. She is a lifelong resident of Booneville.

Office Hours

Our clinic is open Monday through Thursday from 8 a.m. to 5 p.m. and 8 a.m. to noon on Fridays.

Appointments

Appointments may be made by calling (662) 728-2071 during office hours. Appointments may be requested with the health care provider of your choice. Every effort will be made to provide the earliest possible appointment for the convenience of the patient. Due to the unscheduled nature of emergencies, occasional delays do occur. We hope you will understand these delays are unavoidable.

If you are unable to keep your appointment, please cancel as far in advance as possible. Another patient who can be booked into the open time will be grateful for your thoughtfulness.
