Diabetes Medication Policy

Radiology Nursing shall incorporate routine questioning of patients for contrast media patients to determine diabetes and the use Avandamet, Glucophage, Glucophage XR, Glucovance Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Kombiglyze XR

Avandamet, Glucophage, Glucophage XR Glucovance Metformin, Metaglip, Fortamet, Riomet, Actoplus Met, Janumet, Prandimet and Glumetza, Kombiglyze XR Policy

Inpatients and ER patients:

Routine, non-emergency and emergency procedures can be performed by discontinuance of Avandamet, Glucophage, Glucophage XR, Glucovance, Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Riomet and Kombiglyze XR at the time of procedure and waiting 48 hours before continuance of any of these medications. A creatinine study should be performed prior to re-continuation of Avandamet, Glucophage, Glucophage XR Glucovance, Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Riomet and/or Kombiglyze XR..

Outpatients:

Outpatients for contrast procedures that are diabetic and on Avandamet, Glucophage, Glucophage XR Glucovance, Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Riomet and/or Kombiglyze XR will be identified at the time of scheduling so that the referring physician can manage the discontinuance of these medications. A creatinine lab test will need to be ordered within 48 hours post contrast procedure. Technologists and Radiology Nursing shall incorporate routine questioning of patients for contrast media patients to determine diabetes and the use of Avandamet, Glucophage, Glucophage XR, Glucovance, Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Riomet and Kombiglyze XR medication and document on I.V. contrast media permit forms.

Note: The referring physician is responsible for follow-up creatinine lab work and the continuance of Avandamet, Glucophage, Glucophage XR, Glucovance, Metformin, Metaglip, Fortamet, Actoplus Met, Glumetza, Janumet, Prandimet, Riomet and Kombiglyze XR.
