Services
A licensed social worker is available and provides the following services:

Assesses social and emotional factors related to the patient's illness and treatment
 
Collaborates with the patient's physician and treatment team to develop the plan of care
 
Serves as a liaison between the patient/family, treatment team and payer
 
Helps orient the patient and/or family to the treatment program and coordinates family conferences and education
 
Provides counseling and short-term individual/family/group therapy to help strengthen the family/support system and resolve any conflicts related to chronic disabling conditions and pain management
 
Refers patients and their families to community resources for education and assistance
 
Works to resolve abuse or neglect, inadequate food and medical supplies, transportation and other problems that endanger a patient's mental and physical health
 
Verifies a patient's insurance benefits, helps with the pre-certification process for third party payment, and assists with utilization review of program services to the insurance company

Referrals
The NMMC Outpatient Rehabilitation Center staff refers patients to the social worker if issues are identified or if there are concerns.  Patients and families interested in social services can request an appointment.  A physician order for services is not needed.  Services are provided free of charge.
