What to Expect

What to expect during a Skilled Nursing Facility stay

Medical Services:
• You will receive visits by our medical director or nurse practitioner as needed during your stay depending upon your medical condition. If at any time you have additional questions or would like to see them, please let your nurse know.
• Your hospital physician or surgeon does not usually visit during your Skilled Nursing Facility stay.
• Your nurse will schedule follow-up appointments with your primary care physician or surgeon and provide any needed prescriptions at the time of discharge.
• Follow-up appointments and prescriptions will be given to you on your discharge day. This information can be found on your discharge summary.

If you have any questions regarding your discharge services, insurance concerns or financial concerns after discharge, do not hesitate to contact the social worker. Your expected length of stay is usually two to three weeks depending on your progress and health condition, which will be reviewed by your care team each Wednesday. The social worker will keep you updated concerning your discharge date and will write it on the white board in your room. If you have any questions regarding your discharge date, please contact the social worker. Your safety and success after discharge is our goal; please contact the social worker if you struggle with any concerns which may put your health at risk upon discharge.

Group Activities:
Social activities greatly affect our mood and wellness. We offer a wide range of fun activities at 11 a.m. daily in our front lobby across from the nurses’ desk. We play games such as Bingo, Uno, Trivia and various reminiscent activities so you can pass on your experiences with our staff. You are also invited to sit in our lobby during mealtimes to socialize with your family or others. An activity calendar is posted in your room behind the door.

Therapy Services:
Therapy services are scheduled in intervals between 8 a.m. and 4 p.m. Monday through Friday. While we encourage family and friends to visit with patients, we must restrict visits during therapy session times. If visitors arrive during the therapy session, we will ask them to wait in your room. Also, if you receive a phone call, we will ask to take a message and will leave a note on your door advising you of a missed call. I realize this can be difficult for families; however, we ask this in order to help patients reach their highest level of functioning so they can return home safely.

• The nursing staff will be administering your scheduled medications, pain medications and IV medications according to your physician’s orders.

• The nurse will perform treatments for surgical sites or wounds and will keep your physician updated.

• Always call for assistance prior to transferring, walking, standing and especially when bending over to pick up an object. Most falls occur during these activities.

• Pain medications are ordered by your physician on an as needed basis. Please call your nurse if you are having pain. The nurses will ask you the location of your pain and will ask you to rate your pain on a scale from 1 to 10. By rating your pain, we are able to monitor the effectiveness of pain treatment.

• Meals are served at 8 a.m., noon and 5 p.m. If you have not received a meal tray within 15 minutes of these times, please alert the nursing staff by pressing your call light or by dialing 8101 on your hospital phone.

• Staff will be monitoring your weight weekly unless otherwise ordered by the physician. We are able to do this by utilizing your bed-scales. This is scheduled every Wednesday between the hours of 5-7 a.m.

• Showers are scheduled every other day starting on the first day following your admission.

• Bed linen changes: Bed linens are changed once weekly and as needed on Mondays (even room numbers) and Tuesdays (odd room numbers).

• Snacks are available in the Nutrition Room at the nurses’ desk: flavored gelatin, pudding, ice cream, peanut butter, crackers, graham crackers, cheese/crackers, soups (tomato, cream of chicken, chicken noodle and vegetable beef)

• Beverages available : Orange juice, grape juice, cranberry juice, apple juice, soft drinks, milk, chocolate milk, skim milk, butter milk, 2% milk and coffee.

• A registered dietitian will be following your meal plan and intake. Please let us know if you would like to meet with her or if you would like to request a diet change.

• Recommended clothing: Please wear clothes that are easy to put on and take off, such as those with elastic waistbands, button-up shirts, T-shirts, pajamas or gowns, a house coat or robe, etc.

• Footwear: Tennis shoes with rubber soles and house shoes with rubber soles.
