GUEST FEES & TEMPORARY MEMBERSHIPS

FEE $5.00------ Guest Must be with a member

FEE $10.00-----Guest may come in without a member

FEE $25.00-----One week guest pass

FEE $55.00---------Temporary membership for one month
(See guidelines below)

FEE $165.00--------Temporary membership for three months

FEE $42.00---------Temporary pool Membership for one month

FEE $126.00--------Temporary pool membership for three months (See guidelines below)

FEE $99.00---------PACE

Temporary Membership Policy
There are 3 types of Temporary Wellness Center memberships that are currently available:

Type A	College Student/High School Memberships
	Christmas break and summer break, and any other time approved by Member Services Coordinator.
Type B	NMMC Sponsored Physicians Interns
	Must be sponsored by NMMC physician, and approved by Member Services Coordinator.
Type C	Other temporary memberships as approved by the Member Services Coordinator
	(...interns at other businesses, consultants, military, etc.)

	If a person is here visiting and wants to work out, they may do so by going through the normal temporary membership process and present identification showing permanent address, which must be more than 35 miles away.

1	Terms of temporary memberships are dependent upon needs of temporary member, no membership less than one month or more than (3) three months.
2	Dues for all temporary memberships are to be paid in full at the time of enrollment (no pro rate). Any exceptions should be cleared by the Member Service Coordinator.
3	All temporary members must sign a temporary membership agreement.
4	All temporary members must complete the Guest Health History Questionnaire. As with guest, this must be reviewed by a Fitness Instructor (along with having their blood pressure taken).
5	Temporary members should receive a brief Wellness Center tour if time allows.
