2020 Outcomes Logos

Program Information

It is essential for all health care providers, especially those delivering care daily in a clinic setting, to have knowledge of caring for the health of the people in our region. Continuous development of clinical evidence and revisions of existing guidelines requires constant learning for providers. Additionally, communication between the primary care provider and the specialist is vital in order to streamline care, optimize resources, incorporate current guidelines and avoid overlap which results in dangerous medical outcomes and increased costs. These topics will address gaps in fragmented care, gaps between usual and best care and overall lax in coordination of care. The format of this learning is a virtual offering that specifically targets primary care physicians and advanced practice clinicians.
