- Copies of any recent medical records, consultation reports or radiology images
- Name of your pharmacy in case a prescription is needed

Not sure if you need Urgent Care or the ER? Call 1-800-882-6274 anytime to speak directly to a registered nurse and get immediate answers. Using computerized medical protocols, nurses direct callers to the most appropriate treatment. Our nurses are available 24 hours per day, seven days per week.
The Desk is our free community and provider information service. Our call center specialists can help you book appointments and answer your questions about NMHS programs and services. Available 8 a.m.-4:30 p.m. Monday through Friday.
