Your primary care provider is your trusted partner in maintaining and improving your health through preventive care, wellness screenings and management of chronic conditions.

  • Preventive Care. Your PCP can identify and address potential health issues before they become more significant problems. Regular check-ups, routine screenings, immunizations and lifestyle counseling are part of the care your PCP can provide.
  • Coordination of Care. Your PCP serves as the team captain for your health, ensuring everyone on your care team are connected. Your PCP can make referrals to specialists and manage your medications.
  • Building a Relationship. Your PCP knows you and your health history so he or she can recognize potential problems sooner and offer personalized care for you.
