Thank you for your interest in serving on our Patient and Family Advisory Councils. Our goal is to recruit individuals who will bring their expertise to the councils and serve as a voice for patients, family members and the community. Please note the following expectations for this committee. Each applicant must:
- complete onboarding requirements
- adhere to a confidentiality agreement
- attend 3 meetings annually
- be willing to serve a minimum of one year with an option of requesting an additional one-year term
