A quarterly collaboration of health care professionals who serve patients and families following a hospitalization. We welcome team members from skilled nursing, swing bed, rehabilitation, home health and long-term care organizations to discuss changes in health care, review governmental regulation changes and share best practices across these facilities. ACO data, regional targets and key metrics are also shared. You do not have to be an ACO participant to join us.
