North Mississippi Medical Clinics Honor Graham as Star of the Month

Award presentationJamie GrahamTUPELO, Mississippi—North Mississippi Medical Clinics recently named Population Health manager Jamie Graham, RN, as the Star of the Month.

Graham joined North Mississippi Health Services in 2007 as a registered nurse with North Mississippi Medical Center Home Health. In her current role, Graham works with a team to help people better control chronic conditions like high blood pressure and diabetes. She is based at Okolona Medical Clinic, helping Medicare patients navigate the health system and assisting with quality improvement efforts.

“The best part of my job is providing education to my patients and seeing the education and lifestyle changes reflect in their health,” Graham said. “I get to share in their excitement and celebrate their accomplishments.”

In nominating her as Star of the Month, Graham’s colleagues lauded her dedication and compassion.

“Her many years of experience as a home health nurse help her relate to patients and meet them where they are in her current role as a population health manager,” one teammate wrote. “We are thankful for the drive and focus she has with caring for the patients she serves.”

Another coworker wrote, “She is a calm presence in every situation and just jumps in wherever needed.  She never complains and is always very appreciative.  She is always the first to volunteer to help others and has such a caring, giving heart.”

Graham graduated from Vardaman High School and earned her associate degree in nursing from Holmes Community College in Grenada.

She and her husband, Chris, have been married for 21 years and live in Vardaman. They have two sons, Connor and Tucker. The Grahams attend Poplar Springs Baptist Church in Vardaman.
